Room Use Request Form - Recurring Events
- Requests for space should be requested as far in advance as possible to ensure our ability to meet your needs. Please make all requests a mnimum of three weeks prior to requested date.
- All questions with the red star must be answered in order to give you a correct quote. The form will not submit unless all questions with a red star are answered.
- The email address for all inquiries is firstname.lastname@example.org
- We will tell you about availability on a date on the phone or by email. However, we will give quotes only after the full form is filled out.
- Events will be put on our calendar after a signed contract and deposit are received.
- When we receive your form, you will receive notification of availability of date and a cost estimate.
- Please note: this is not a contract.
